Financial Aid Request
The form below is for those who would like financial assistance with items we put on the budget this year; they are:
Technology Upgrades - includes cameras and equipment for live or recording service
Pastoral Health / Counseling - this would be for pastors and wives who need either counseling or mental health assistance. Could also be used for a retreat getaway weekend, for refreshing or renewal. Such as a marriage retreat weekend.
Student Discipleship - Goes towards fees for students who are taking courses while in school towards leadership training for the local church where they attend.
Pastoral Training - Goes towards Pastors and lay leaders in your church who are taking courses towards pastoral training or continued education.
Financial Aid Scholarship -For youth/students who need assistance with CNBC event registration fees.
If it is for Pastoral health concerns, you may submit the form without all three signatures to keep your request confidential; however, we would encourage you to let someone on your leadership team know of your situation so that they can help you and support you as well.
If you need help in any of these areas just fill out the form and we need three signatures on the form. The pastor, a trustee or board member of some kind such as; elder, deacon etc., and your church treasurer.

Event Trailer
The CNBC Event trailer is for use by our Association of churches. The form below form is used to request the trailer. It has been outfitted with some new items and documents that you need to be accountable for when using it.
The Trailer w/ CNBC Logo contains:
1 - Large Bouncy Pirate Ship with working water attachments
1 Smaller Bouncy house
Complete Sound System with: Board, Active Speakers, Stands, Mics (2)
Inboard Quiet Generator for use with Sound system and sources close to head table
2 Regular Generators for Bouncy Houses
Popcorn machine (Includes pre-measured popcorn and butter/oil packs)
Grill
Tables and Sunshades
The event trailer comes complete with packing instructions and loading diagrams for easy unloading and loading. It is suggested to have at least 4-5 people to help with unpacking and packing trailer..
Please note: The person responsible for the trailer needs to be with whomever picks it up. Documents need to be gone over and signed. If it is being delivered the person responsible needs to be available at delivery.
A Deposit will be required to ensure that the trailer is brought back the way it was delivered to you. Deposit is returned when the trailer is fully delivered with all items intact.
